The Vital Statistics division of the Massillon Health Department provides certified copies of birth and death certificates for Massillon Community Hospital/Affinity Medical Center. Genealogy research is available for records from 1889 to present.

Birth and Death Records

The cost of a Birth or Death Certificate is $25

Records are available from 1909 to the present for births that occurred within
the State of Ohio
and for deaths which occured within the the City of Massillon only.

For Birth Certificates by mail, please send a written and signed request stating the BIRTH name, date of birth, father's name, mother's name at the time of the birth and mother's  maiden name.

For Death Certificates please include the name at the time of death and the date of death.

Please include a stamped return envelope.

Money Orders, Bank Checks and Certified Checks are appreciated.  A personal check is acceptable when accompanied by a clear, readable copy of your valid driver's license.  Please make checks payable to the Massillon City Health Department and mail to:

The Massillon Health Department is happy to announce that we are now able to issue Birth Certificates for any birth occurring within the State of Ohio after 1909.  The Ohio Department of Health recently implemented a program in which qualifying local Health Departments can issue a certified Birth Certificate originating anywhere within the State, using a new web-based program.   This new program makes it possible to purchase a Birth Certificate at your local Health Department instead of making it necessary to travel to the Health Department where the birth occurred or having to request it by mail or through the internet.  We are extremely happy to be authorized to offer this new service.

We do not have Death records for deaths that occurred at Doctors Hospital or nursing homes and hospitals located outside the Massillon city limits. Please contact the Stark County Health Department at 330-493-9904, ext. 0, for further info.  These records may also be ordered on-line at www.vitalchek.com.  In the Event Location section, put the township name (Perry, Jackson, etc.) instead of Massillon, so the request goes to the proper Health Department for processing.

For your convenience, you can request birth and death certificates online through an independent company that the Massillon City Health Department has partnered with to provide  this service called VitalChek Network, Inc.  VitalChek can be reached through their website; www.vitalchek.com.   There is an  additional fee charged by VitalChek for this service, and all major credit cards are accepted, including American Express®, Discover®, MasterCard® or Visa®.”

Birth or death certificates ordered this way are generally mailed out the next business day. Please note on that website that on the Shipping section, United Parcel Service is the delivery method checked, with an extra fee for faster delivery. You may change that selection to Regular Mail, if preferred. Vitalchek can be used to order certificates from most parts of the country.

The Birth Registration Card (wallet-size card) and uncertified copies of birth and death records are no longer available. When requesting information by e-mail regarding birth and death records, please put "birth/death info" in the subject line of your e-mail.

Uncertified copies of birth and death records can no longer be issued.  Genealogy requests are available at the Vital Stastics counter.  Certified copies of birth and death certificates may be purchased for $25 cash, personal check (with a valid driver's license), or MasterCard.
